Commit to Connect is seeking applicants for its newest Community of Practice (COP). Launching in spring 2026, this COP will focus on measuring the social connection impact of Chronic Disease Self-Management (CDSME) programs.

COP members will implement the Upstream Social Interaction Risk Scale (U-SIRS) and the Retrospective Assessment of Connection Impact (RACI) to collect pre- and post- data on CDSME programs. The COP will virtually meet five times between March and September 2026. Members will receive no-cost access to the collection instruments and receive technical assistance from CTC and a leading researcher, Dr. Matthew Lee Smith.
