Join ACL's Commit to Connect initiative on Wednesday, February 11, from 12:00 to 1:00 PM ET, for a webinar on the value of outcome evaluation of social connection within chronic disease self-management education (CDSME) programs.

In 2025, Commit to Connect launched the Understanding the Impact of Social Isolation and Loneliness Programs Community of Practice (COP) to support six community care hubs (CCHs) in conducting retrospective evaluations of social connection outcomes in CDSME programs.

Dr. Matthew Lee Smith, technical lead for the COP, will highlight COP findings during the webinar. Organizations that participated in the COP will also share their best practices on measurement and evaluation and how they are applying findings for future work. The webinar will include brief speaker presentations and a panel discussion, with time for audience Q&A.
